About us
Heritage Assisted Living is a small home in the country. We offer different sizes of rooms and setups to accommodate your needs and budget. Being smaller in size we naturally provide personalized daily cares, genuine friendships and the individualized needs that are often lost in larger facilities. Heritage is owned and operated by a mother and daughter team; Mary Ann Nielsen of Rexburg and Shawna Warner of Rigby. Together they have 20+ years experience, giving residents and their families the peace of mind that they have chosen a family with knowledge, experience and a genuine love to care for those in need of assisted living or in-home care.
Heritage offers a more intimate experience for our residents; flexibility in creative and home cooked meals to meet each persons taste-buds,we provide personalized daily cares without our residents feeling rushed, and we cater to the little details for each of our resident. Something that is so dear and important during our senior years. Heritage knows, understands and offers to each of our residents a "HOME". A home with family, love, compassion,experience and always making sure fun and laughter is part of the day.

Exercise and Activites
Exercise Program
Throughout the week we have multiple organizations come in and do physical activities,whether it’s stretching to get your muscles moving with Rexburg Message Therapy School or whether it’s the Physical Therapist doing exercises. We also organize walking groups when weather allows outside.
Activities
We have activities planned every day of the week and all residents are invited to join and participate if they would like. We can scheduled a stylist to come in and provide haircuts, coloring, perms and shampoo & sets. We have religious organizations come and perform services on a weekly basis, as well as regular Sunday services. Many volunteers from our community come in and spend quality time with our Residents. Often times different groups come in and play games and provide musical programs.
